Photography Consent

I hereby grant Ollie & Friends Pet Sitting permission to use any photographs taken of myself or my pet, and any and all of its publications, including website entries, without payment or any other consideration. I understand and agree that these materials will become the property of Ollie & Friends Pet Sitting and will not be returned. I hereby authorize Ollie & Friends Pet Sitting to edit, alter, copy, exhibit, publish or distribute this photo for purposes of advertisement, publicizing programs or any other lawful purpose.

In addition, I waive any right to royalties or other compensation arising or released to the use of the photograph. I hereby release rights to all claims, demands, and causes to action which I, my hires, representatives, executors, administrators, or any other persons acting on my behalf of my estate, have or may have by reason of this authorization.

Terms & Conditions

1. Ollie & Friends Pet Sitting agrees to provide pet sitting, dog walking and other applicable pet care services in a reliable, caring and trustworthy manner. In consideration of the services and as an express condition thereof, the client expressly waives and releases Ollie & Friends Pet Sitting from any and all claims against the company, its owners, employees and representatives, except those arising from negligence or willful misconduct on the part of Ollie & Friends Pet Sitting.

2. Client agrees to notify Ollie & Friends Pet Sitting of any concerns within 24 hours of completion of services.

3. Client agrees to pay all charges accrued for services rendered. Client understands that payment is due at or prior to the time of the commencement of services unless supplementary payment terms are expressly agreed upon in writing between the parties.

4. Client will incur a $50 charge for each returned check in addition to any and all bank fees attributed to the returned check.

5. Ollie & Friends Pet Sitting shall exercise all precautions against sickness, injury, escape, loss, accidents or death of Client’s pet(s). Ollie & Friends Pet Sitting is not responsible for sickness, injury, escape, loss accidents or death of Client’s pet(s) unless caused through negligence or willful misconduct on the part of Ollie & Friends Pet Sitting.

6. Client represents and warrants that pet(s) are currently vaccinated in accordance with all local, state and federal laws and regulations.

7. Ollie & Friends Pet Sitting will follow the directions of the Veterinary Release Form in the case a pet should become injured or sick.

8. Client accepts responsibility for all medical expenses and other damages resulting from an injury to the Pet Sitter, other persons or other animal(s) caused by the Client’s pet(s) or negligent act.

9. Client agrees to indemnify, hold harmless and defend Ollie & Friends Pet Sitting in the event of a claim by any person injured or otherwise damaged by Client’s pet(s) or negligent act.

10. Ollie & Friends Pet Sitting reserves the right to charge a cancellation fee of 100% of the scheduled visits for services canceled with less than 1 hour's notice prior to the scheduled service.

11. Ollie & Friends Pet Sitting reserves the right to terminate this contract at any time if the Pet Sitter, in his/her sole discretion, determines that the Client’s pet(s) poses a danger to the health or safety of itself, other pets, other people or the Pet Sitter. If concerns prohibit the Pet Sitter from caring for the pet, Ollie & Friends Pet Sitting will attempt to contact the Client to arrange alternative care. If the Client cannot be contacted, the Client authorizes Ollie & Friends Pet Sitting to place the pet in a licensed kennel with all charges and fees arising to be the responsibility of the Client.

12. Ollie & Friends Pet Sitting reserves the right to refuse service to any client, at any time, for any reason.

13. This document gives Ollie & Friends Pet Sitting and its representatives authorization to enter the Client’s listed address as needed to perform agreed upon services.

14. Client expressly gives Ollie & Friends Pet Sitting the authority to employ a locksmith on their behalf and to promptly reimburse Ollie & Friends Pet Sitting for all costs incurred in the event of a malfunction of the lock, keys or automatic door opener.

15. Ollie & Friends Pet Sitting is not liable for any loss or damage in the event of a burglary or other crime that should occur while under this contract.

16. Client agrees to properly secure the home prior to leaving the premises. Ollie & Friends Pet Sitting agree to properly re-secure the home to the best of its ability at the end of each visit.

17. In the case of an emergency, inclement weather or a natural disaster, the Client authorizes Ollie & Friends Pet Sitting to use reasonable judgment for the care and well being of Client’s pet(s) and residence. Ollie & Friends Pet Sitting will make reasonable efforts to maintain service during these conditions but reserves the right to adjust the schedule of service based on the sole discretion of the Pet Sitter. Ollie & Friends Pet Sitting will bring Clients' pet(s) to their home in the case of a natural disaster or inclement weather; blizzard, tornado, etc. Ollie & Friends Pet Sitting is not liable for any injuries or death of Clients' pet(s) due to a natural disaster or weather emergency.

18. Ollie & Friends Pet Sitting is not responsible for any damages beyond the control of the Pet Sitter.

19. Client is responsible for supplying the necessary equipment and supplies needed for the care of their pet(s) including, but not limited to, a sturdy, well-fit harness or collar and leash for walks or in case of emergencies, pet food, medications, identification tags, litter boxes, cat litter and cleaning supplies. Client authorizes the purchase of any necessary equipment or supplies for the satisfactory performance of duties. Costs of all purchases and related service fees will be reimbursed to Ollie & Friends Pet Sitting within 10 days.

20. Client authorizes this contract to be a valid approval for services to permit Ollie & Friends Pet Sitting to accept all future in person, telephone, online, mail or email reservations and provide services without additional signed contracts or written authorizations. Should Ollie & Friends Pet Sitting revise contractual documentation, the revised form will need to be reviewed and signed prior to future services.

21. The Term of this document apply to all pets owned by the client, including any and all new pets that the customer obtains on or after the date this document was signed.

Inclement Weather Policy

Ollie & Friends Pet Sitting and their associates are here to provide you and your pet(s) the best experience. The following policy addresses the procedure Ollie & Friends Pet Sitting and their associates will take in the event of any unpredictable weather that occurs in the Midwest. If at any time inclement weather becomes a factor on the performance of the agreed upon service, this policy shall provide notice to the client that Ollie & Friends Pet Sitting and their associates shall take reasonable measure as outlined in this policy to perform the agreed upon service.  “Inclement Weather” shall refers to forms of extreme heat, snow, rain, icy walking conditions, extreme wind and/or wind gusts, lightening and/or thunder within a 15 mile radius of the designated walking area, and any other reasonable conditions that shall be deemed inclement by Ollie & Friends Pet Sitting. 

All temperature readings shall be made from the Fahrenheit scale and read at the whole number temperature. If at any time the temperature shall read with a decimal (example: 50.1 degrees), the temperature shall be rounded up to the nearest whole number. When additional weather factors contribute to the temperature (ie. It is 35 degrees with wind that makes it feel like 25 degrees, or something of the like), Ollie & Friends Pet Sitting shall determine the course of the service to be performed depending on the present and ‘feels like’ temperature. 

When any agreed upon service is adjusted due to any weather deemed to be inclement by Ollie & Friends Pet Sitting, the agreed upon service shall include a bathroom break and be substituted by one of the following: 

• Shortened Walking Time

• Indoor Play 

• Indoor Training 

• Indoor Brain-Stimulating Activity

• Any service which Ollie & Friends Pet Sitting deems to be substantially similar to that of the agreed upon service

The following shall set out Ollie & Friends Pet Sitting Weather ranges and policy: 

95° or higher – When the temperature of the designated walking area is 95 degrees or higher, Ollie & Friends Pet Sitting and their associates will consider the heat inclement weather and will perform services based on the health and well-being of the pet(s). 

40° to 95° – When the temperature of the designated walking area is between 40 degrees to 95 degrees without any inclement weather, Ollie & Friends Pet Sitting and their associates will perform the agreed upon service. 

32° to 39° – When the temperature of the designated walking area is between 32 degrees to 39 degrees or the wind while wind gusts are under approximately 15 mph, Ollie & Friends Pet Sitting shall perform no walking service over a time frame of 15 minutes and the agreed upon service shall be substituted. 

32° or below – When the temperature of the designated walking area is below 32 degrees and/or has any inclement weather present, Ollie & Friends Pet Sitting and their associates shall not perform a walk and substitute the agreed upon service.
